The Polyclinic Family Medicine Department provides patients with high-quality, personal, and comprehensive family medicine care. Our physicians are trained to provide acute illness care, chronic illness care and preventative care. Family Medicine delivers continual medical care through all stages of patients’ lives, which in essence builds a strong relationship of trust and reliability with our patients. Important Patient Reminders

  • Patients are responsible to follow-up on any tests, imaging, or specialists’ referrals ordered by their family physician. For continued medical care we strongly recommend scheduling a follow-up appointment with a family doctor to discuss the results.
  • Referrals to specialists are not made simply upon patient request. A physician is required to assess the patient’s medical situation, and based on the findings make an appropriate medical decision or if necessary order additional testing before referral to specialist may be made. Referrals to specialists will be based on if medically required in the determination of the referring physician.
  • There are services that are not covered by OHIP (Ontario Health Insurance Plan). For example, missed appointment fees, phone advice, prescription renewals by fax or phone, work or school forms, and sick, massage, orthotics notes. Fees for out-of-OHIP services are charged per Ontario Medical Association Guidelines.
  • Please be respectful. We have a zero-tolerance policy for verbal abuse (yelling, scolding or using inappropriate language). Polyclinic staff are all here to help you receive the best medical care in a timely manner. If there is a concern, please ask for the clinic manager who will be happy to assist you.
  • Refills of narcotic, sedative, or stimulant prescriptions will be made only by originally prescribing physician.

Patient Recommendations:

  • Please choose one pharmacy location for all your prescription drugs. This helps prevent drug errors and provides you extra continuity of care.
  • Please be considerate of other patients waiting to be seen. Try to keep the purpose of each visit to a maximum of 2 concerns. If this is not possible, please advise staff in advance when booking your appointment.
  • Teenagers are encouraged to be seen individually and apart from their parents to improve adolescent care. Due to the confidential nature of patient visits, physicians are unable to disclose details without the teenager’s consent.
  • Patients may always request to be accompanied by a family member or Polyclinic staff for sensitive physical exams such as vaginal, breast, rectal or genital exams.

The Polyclinic health team always strives for 100% patient satisfaction. However if you are dissatisfied with the medical care provided, please respectfully discuss your concerns openly with your physician as soon as the issues arise. You may also discuss the issue with our clinic manager. Both patient and doctor have the right to end a doctor-patient relationship on a reasonable basis. If the physician initiates dismissal from the practice, the patient will receive a formal letter advising of the reason and will have 30 days of continued medical care from the physician, as well as access to our walk-in services.

If you cannot make your appointment on time or must cancel for whatever reason, please inform us immediately by calling 416-222-6160.  In the event we do not answer, while we are busy serving another patient, please leave a detailed message and one of our secretaries will be sure to return your call.  All messages are checked regularly throughout the day, however, please give us up to 2 business days to respond to messages.

Appointment Instructions:

  • Must bring a VALID HEALTH CARD and ALL MEDICATIONS you are currently taking.
  • If Health Card is not valid or expired, uninsured visit charges will apply.
  • Patients who wish to RESCHEDULE or CANCEL their appointment must do so at least 48 business hours in advance to avoid a NO-SHOW fee of $40.
