Appointments for any specialty physician at the Polyclinic are available on a referral basis only. If you wish to have an appointment please have your doctor fill out one of the referral forms and fax it to us prior to booking any appointment. All department fax numbers can be found here.
Patients are required to provide 2 business days prior notice should they wish to reschedule or cancel their appointment.
For Fibroscan 5 business days prior notice is required.
