Polyclinic Specialty Medicine was created for patients’ convenience and for comprehensiveness of medical services offered. Polyclinic patients have the advantage of accessing specialists in various medical fields at the same location as their family physicians.  Nevertheless, Polyclinic Specialty Medicine welcomes all patients regardless of their referring physician.  Completed Referral forms must be faxed to 416-645-1978.

In order to be seen by a specialist, patients must first present a referral form issued by the patients’ referring physician. The referral must have been issued within the past 12 months.  Each referral received at Polyclinic Specialty Medicine is screened by a specialist to determine if additional medical information from the referring physician is required, this might include investigations such as recent bloodwork, any imaging related to the diagnosis, or other specialist reports.  In some instances, patients will be required to bring CDs of X-Rays, MRIs, or any other imaging related to the diagnosis if the specialist finds it necessary.  Failure to follow instructions may result in your appointment being re-scheduled.

Reminder List:

  • Must bring a VALID HEALTH CARD and ALL MEDICATIONS currently taking.
  • If Health Card is not valid or expired, uninsured visit charge(s) will apply.
  • Must bring a translator if the patient cannot speak English
  • Patients who wish to RESCHEDULE or CANCEL their appointment must do so at least 48 business hours in advance to avoid a NO-SHOW fee of $200.

Currently, the Polyclinic Specialty Medicine houses over 40 specialists in a wide array of medical specialty areas, as well as provides specialty diagnostic testing and procedures.

Each referral received at Polyclinic Specialty Medicine is screened by a specialist to determine if additional medical information is required, which might include investigations such as recent blood work, any imaging related to the diagnosis, or other specialist reports. Each patient must ensure the referring physician’s office had included all the necessary test results when the referral is being faxed to the Polyclinic Specialty Medicine. In some instances, patients will be required to bring CDs of X-Rays, MRIs, or any other imaging related to the diagnosis if the specialist finds it necessary. Failure to follow instructions may result in your appointment being rescheduled.Initial consultation with a specialist is scheduled on a referral basis only when the referral letter is faxed to the Polyclinic Specialty Medicine at 416-645-1978.
